Since we found a Phorum bug in the Facebook thread, I thought I'd mention another really conspicuous and memorable programming error I saw (I'm referring to ones I've seen in person; The Daily WTF has a whole collection of others).

My brother and I used to stay up late on New Year's Eve. We were watching the Times Square celebration on I believe it was ABC on December 31, 1999. ABC had a countdown timer displayed on the screen (in some horrible font like Comic Sans, which is a bit of a WTF in itself), and it would count down like this:
4:03
4:02
4:01
4:00
3:60
3:59
In other words, they were adding an additional second every minute.

I pointed this out to Mark, my brother, and triumphantly said "I've found the Y2K bug!"

(They fixed the clock before the crucial countdown began)

If you don’t have freedom as a principle, you can never see a reason not to make an exception. There are constantly going to be times when for one reason or another there’s some practical convenience in making an exception.

I don't have anything to contribute, so I made this.

I hope the conspicuousness and comicsansicalness is satisfactory.
